This study aims to analyze the effect of religiosity on employee performance mediated by empathy at Puskesmas Paninggahan, Solok Regency. Employing a quantitative causal approach, the research involved all 45 health center employees as respondents using a total sampling technique. Data were collected via structured questionnaires and analyzed using Partial Least Squares Structural Equation Modeling (PLS-SEM). Hypothesis testing reveals that religiosity has a positive and significant effect on employee performance and empathy. Empathy also demonstrates a positive and significant influence on performance. The indirect effect test confirms that empathy significantly mediates the relationship between religiosity and performance. The model exhibits strong predictive power with for performance and Q²predict. The study concludes that internalizing religious values enhances performance both directly and indirectly through strengthened empathetic capacity. Practically, health center management should integrate workplace spirituality development and empathy training as a holistic strategy to optimize primary healthcare service quality in rural areas.
