Perineal trauma is a common morbidity associated with vaginal birth and may affect women’s postpartum health and quality of life. Adequate antenatal education is therefore needed to improve pregnant women’s understanding of perineal trauma prevention, including antenatal perineal massage. This community service activity aimed to provide evidence-based education on perineal trauma and antenatal perineal massage and to describe changes in third-trimester pregnant women’s knowledge scores before and immediately after the educational activity. The activity was conducted at Papilio Maternity Home, Sidoarjo, on November 16, 2025, involving 20 participants. An educational approach combining an interactive lecture, demonstration, guided practice using a perineal model, discussion, leaflet distribution, and an instructional video was implemented. Knowledge was evaluated using a one-group pre-test–post-test design. The mean knowledge score increased from 7.90 before education to 9.00 immediately afterward, representing an increase of 1.1 points or 13.9% relative to the baseline mean score. These descriptive findings indicate an immediate improvement in participants’ knowledge following the educational activity. However, the absence of a control group and long-term follow-up precludes conclusions regarding causal effectiveness, knowledge retention, behavioural change, or reduced perineal trauma during birth. Integrating evidence-based perineal health education into antenatal care and pregnancy classes is recommended, accompanied by longer-term evaluation of knowledge, practice, and birth outcomes
