Urban infrastructure reporting still faces challenges related to unstructured reporting channels, difficulty in specifying locations, limited visual documentation, and insufficient follow-up transparency. This study aims to design and evaluate an urban infrastructure reporting interface using the User-Centered Design (UCD) approach through the Understand, Specify, Design, and Evaluate stages. User needs were identified through exploratory interviews and observation and were then translated into user personas, user journey maps, information architecture, and high-fidelity prototypes. Usability evaluation was conducted with 40 respondents using the System Usability Scale (SUS). The prototype integrates road, drainage, and public facility damage categories, photo documentation, map-based location, and report status monitoring. The evaluation yielded an average SUS score of 74.63, with a median of 73.75 and a score range of 52.50–100.00, indicating a positive usability perception. The evaluation also identified areas for improvement in initial use, reporting access, information density, progress transparency, and interface responsiveness. This study contributes by integrating urban infrastructure damage reporting needs into a single UCD-based interface flow validated through user evaluation.
